Who is responsible for your information?
Dr Helen Blake is the data controller for personal information processed through this independent psychology practice. Enquiries can be sent to enquiries@drhelenblake.co.uk.
Information that may be collected
Depending on the service, this may include contact details, enquiry information, appointment and payment information, clinical records, correspondence, outcome measures, referral information and information required for professional, legal, safeguarding or insurance purposes.
How information is used
Information is used to respond to enquiries, assess whether a service may be appropriate, provide psychological services, maintain records, communicate within agreed professional arrangements, manage payments and comply with professional, legal and safeguarding responsibilities.
Initial emails
Please do not include detailed trauma histories, medical records, legal bundles or highly sensitive information in an initial email. Where a professional referral appears suitable, a secure method for agreed documentation will be provided.
Lawful basis and health information
The lawful basis depends on the context and may include steps taken at your request before entering a contract, performance of a contract, legitimate interests, legal obligations and protection of vital interests. Health and other special-category information is processed where necessary for health or social-care purposes by a regulated professional, or where another lawful condition applies.
Sharing information
Information is not sold. It may be shared where necessary and lawful with professional referrers or funders within agreed consent arrangements, clinical supervisors under confidentiality, service providers supporting secure practice systems, or statutory bodies where required. Information may also be disclosed where there is a serious safeguarding concern, an immediate risk of serious harm or another legal requirement.
Retention
Information is kept only for as long as necessary for the purpose for which it was collected and to meet professional, legal, insurance and accounting obligations. The applicable clinical-record retention arrangements are explained in the practice terms. Unsuccessful or discontinued enquiry information is reviewed and deleted when it is no longer reasonably required.
Security
Proportionate technical and organisational measures are used to protect information. No email or internet service can be guaranteed to be completely secure, so sensitive information should only be sent using an agreed method.
Your rights
Depending on the circumstances, you may have rights to access, correct, restrict, object to or request deletion of personal information. Some rights are limited where records must be retained for professional or legal reasons. Concerns can be raised directly with Dr Helen Blake or with the Information Commissioner’s Office.
Website hosting
This website is hosted by Netlify. Basic technical logs may be processed to deliver and protect the site. The website does not use advertising trackers, behavioural profiling or a clinical enquiry database.
